Notice of Election
This is the official ballot for the run-off election of Officers for Communications Workers of America Local 4502.
The election is being conducted for the offices of President, Vice President and Secretary. The official ballot is
accompanied on the reverse side of this notice. The enclosed ballot must be returned by United States mail
and must be received by 8:59 a.m. on January 6, 2012. Ballots will be counted on:

A vote cast for the candidate for the office of President, Vice President, and Secretary shall also constitute a
vote for such candidate to serve as a delegate to the CW A conventions held during the term of office for which
the election is being conducted, in the event such candidate is elected President, Vice President, and
Secretary.
Enclosed please find:
• One ballot.
• One ballot envelope.
• One return addressed, postage-paid mailing envelope.
Voting Instructions:
• Mark an "X" in the box next to the name of the candidates of your choice.
• Fold and place the ballot in the ballot (plain) envelope and seal it. Do not mark this envelope in any
way.
• Place the sealed ballot envelope in the postage paid return addressed envelope, seal it and affix YOUR
NAME and address on the upper left hand corner of the return addressed envelope.
• NOTE: Do not remove your name label from the back of the stamped return envelope. Your name on the address label will identify you as an eligible voter. Your ballot cannot be counted unless this named label is checked against the membership eligibility list. It will not reveal how you voted, because the sealed ballot envelope is removed and separated from this envelope before it is opened. Secrecy can be preserved only if you mark and mail your ballot personally. Do not allow someone else to mark and handle your ballot or see your vote. Mail your ballot so that it will be postmarked and delivered to the post office box not later than 8:59 am, on Friday, January 6, 2012.

RUN-OFF ELECTION RULES FOR CANDIDATES
1. Candidates may review the list of members in good standing during normal business hours. The membership list, including addresses of members, may not be copied in whole or part by a candidate.
2. City equipment and property may not to be used to campaign. This includes copiers, faxes, printers, computers, voice mail, e-mail, inter-office mail, conference rooms, offices, phones, bulletin boards and other items that may not be listed.
3. You may not post any items in any City building or on City property.
4. Campaigning in work areas during working hours is prohibited. Doing so may be viewed as disrupting the workplace and violate City Work Rules.
5. You are not permitted to use the CWA Local 4502/CMAGE or CWA logo on any website, literature or envelopes with regard to your candidacy
6. A union or employer may not contribute money or anything of value (such as the use of facilities, equipment, or supplies) to promote the candidacy of any individual in this election.
A. The prohibition against the use of union and employer funds applies to any union and any employer, not just CWA Local 4502/CMAGE or an employer of the CWA Local 4502/CMAGE’s members. For example, it is improper for a candidate to have campaign literature duplicated free of charge on a copy machine at a small business owned by a relative of the candidate.
B. Any expenditure of union or employer funds in support of or opposition to a candidate, even if the amount is small, is a violation of these Election Rules for Candidates.
C. The prohibition against the use of union and employer funds applies to direct expenditures from the union or employer as well as indirect expenditures including:
(i) Campaigning on time paid for by the union or employer;
(ii) Use of union/employer owned or leased equipment such as telephones, fax machines, and copy machines;
(iii) Use of union/employer supplies such as stamps, paper, and envelopes;
(iv) Use of union employees to prepare campaign literature while on union time;
(v) Use of union letterhead;
(vi) Use of union/employer property or facilities;
(vii) Printing articles which support or criticize an individual’s candidacy in a union newspaper or other publication; and
(viii) Giving free services or special discounts to a candidate customer such as printing, photocopying, etc.
7. The Election Committee and its designees will mail campaign material to eligible voters on behalf of candidates. The Election Committee will notify all candidates of the total number of eligible voters. A candidate must provide the Election Committee with postage pre-paid envelopes containing the campaign material to be mailed. An additional copy of the campaign material must be submitted to the Election Committee for their records. The campaign materials must be delivered to CWA Local 4502/CMAGE, Attention: Deborah Ioia, Chair, Election Committee, 1350 W. 5th Ave., Suite 300, Columbus, Ohio 43212. The Election Committee will affix mailing labels to the campaign envelopes and deposit the envelopes at the United States Post Office for delivery. Mailing labels will not be provided to candidates. Campaign materials received by the Election Committee after 5:00 p.m. on December 20, 2011 will not be mailed. No other method of mailing campaign materials to eligible candidates is permissible under these Candidate Election Rules.
Candidates must pay $100 for each mailing. The material expense for labels and printing will be $10. The City charges a released member’s pay plus a markup for fringe benefits of $30 per hour. It will take close to 3 hours of labor to affix the labels, along with the time and gas cost to deliver the mailings to the post office. Participating candidates must submit a check payable to CMAGE/CWA Local 4502 in the amount of $100.00 at the time the campaign materials are submitted for mailing.
8. All candidates must provide the Election Committee with the following contact information: Name, Mailing Address, and Telephone Number where you can be reached.
9. Candidates are authorized to have an observer, who must be a member in good standing, present when ballots are prepared for mailing on December 22, 2011, and are collected from the post office and counted on January 6, 2012.
10. Ballots will be mailed to eligible voters on December 22, 2011. Ballots will be counted by the Election Committee on January 6, 2012 beginning at 9:30 am at CMAGE/CWA Local 4502, 1350 W. 5th Avenue, Suite 300, Columbus, Ohio 43212.

Election Information
The official ballot for the election of Officers for Local 4502, Communications Worker of America will be mailed by November 16th from Illinois. The election will determine the offices of President, Vice-President, Secretary, Treasurer and Board of Trustees. The official ballot must be returned by United States mail to the Election Committee post office box by 8:59 a.m. on December 5, 2011. Ballots will be counted on December 5, 2011, at the Union Hall, 1350 West 5th Avenue, Suite 300, Columbus, Ohio; beginning at 9:30 a.m.. Instructions for completion of the ballot and its return are printed on the reverse of the ballot. Please be sure to follow all instructions. 9. All candidates must provide the Election Committee with the following contact information: Name, Mailing Address, and Telephone Number where you can be reached.
10. Candidates must inform the Election Committee how their name should be stated on the Official Ballot no later than Friday, October 28, 2011. If a candidate does not provide such information, the candidate’s name as it appears on the official membership list will be used.
11. Candidates are authorized to have an observer, who must be a member in good standing, present when ballots are collected from the post office and counted.
