Dependent Eligibility Forms Due !!

PLEASE NOTE THAT ALL ELIGIBILITY FORMS ARE DUE BEFORE THE END OF JANUARY!!

Local 4502 has just learned that the new Federal Healthcare Reform law has eliminated and changed some of the eligibility requirements for dependent children between 19 - 26 years of age. The new Federal legislation requires only that dependent children between the ages of 19-26 not work for an employer that offers health coverage.

What this means to ALL City employees who are wanting to cover any dependent children between 19-26 years of age....regardless of whether they have previously been documented and submitted to the City - will require you to re-submit a new affidavit of coverage to your HR/Payroll representative.

Please use the affidavit form that we posted on the January 3rd article to re submit your dependent childs affidavit prior to the end of January. If you have any questions, you can ask your appropriate HR representative. LOCAL 4502 STRONGLY ENCOURAGES YOU TO SUBMIT THAT AFFIDAVIT TODAY TO ASSURE YOUR ELIGIBLE CHILD IS PROPERLY APPLIED FOR!!
