New Insurance Open Enrollment Now Open! Dear Local 4502 Member, In compliance with the federal healthcare reform, the State of Ohio House Bill 1, and Council Ordinance 1077-2010 guidelines, the City will be conducting an open enrollment period during the month of January to allow employees to enroll their adult children and eligible dependents to the City’s health plan. The insurance effective date will be February 1, 2011. I have included the information and enrollment forms for your reference. The attached letter and dependent checklist were mailed to all city employees over the weekend. We have also attached affidavit forms that will be available in the division’s human resources office. Employees are encouraged to call the EBRM hotline at 645-8624 should they have questions or need clarification on the enrollment process.
