Notice of Election

This is the official ballot for the run-off election of Officers for Communications Workers of America Local 4502. The election is being conducted for the offices of President, Vice President and Secretary. The official ballot is accompanied on the reverse side of this notice. The enclosed ballot must be returned by United States mail and must be received by 8:59 a.m. on January 6, 2012. Ballots will be counted on:

Location: CWA Local 4502 Union Hall, 1350 West 5th Avenue, Suite 300, Columbus, OH 43212

A vote cast for the candidate for the office of President, Vice President, and Secretary shall also constitute a vote for such candidate to serve as a delegate to the CW A conventions held during the term of office for which the election is being conducted, in the event such candidate is elected President, Vice President, and Secretary.

Enclosed please find:

One ballot.
• One ballot envelope.
• One return addressed, postage-paid mailing envelope.

Voting Instructions:

• Mark an "X" in the box next to the name of the candidates of your choice.
• Fold and place the ballot in the ballot (plain) envelope and seal it. Do not mark this envelope in any
way.
• Place the sealed ballot envelope in the postage paid return addressed envelope, seal it and affix YOUR
NAME and address on the upper left hand corner of the return addressed envelope.
• NOTE:   Do not remove   your name label from the back of the stamped return envelope. Your name on the address label will identify you as an eligible voter. Your ballot cannot be counted unless this named label is checked against the membership eligibility list. It will not reveal how you voted, because the sealed ballot envelope is removed and separated from this envelope before it is opened. Secrecy can be preserved only if you mark and mail your ballot personally. Do not allow someone else to mark and handle your ballot or see your vote. Mail your ballot so that it will be postmarked and delivered to the post office box not later than 8:59 am, on Friday, January 6, 2012.
